Clinic is open to any player, boy or girl, in the 4th-8th grade. Players will have an opportunity to learn the skills of volleyball. Depending on number of participants, players will be divided into groups based on age and skill level. Beginners and experienced players are welcome! The registration fee to participate is $15 per person.
Submission of this form is agreement that the Player named above will participate in a volleyball clinic at Wootton High School. The above-named Parent understands that volleyball is an athletic activity and has inherent risks. The Parent releases and discharges the Activity Provider, Provider’s employer, spouse, heirs, and administrators from all manner of legal or financial action related to the clinic. The Parent agrees to accept all liability for injuries which may occur during the event. The Parent further attests that the Player named above does not have any physical or mental disabilities or medical ailments that would limit or prevent the Player from participating fully in the activities of the clinic.
The fee for attendance is $15. The Parent agrees that under no circumstances will a refund be provided even if the Player is unable to attend or complete the activity.
